It can be difficult and time consuming to assist consumers moving back into the community from nursing facilities and other institutions. But don’t let that stop you from doing it!
Successful transitions from institutions back to the community often require coordination of all of your CIL’s core services. IL skills are especially important to make sure consumers that move home – stay there and live happily and independently. Our presenters will explain how to apply what you already know and how to expand on your IL skills training specifically for individuals who are moving to the community. Find out how you and your staff can assist consumers.
Target Audience
Executive Directors, Directors of Independent Living Services, and direct service staff of Centers for Independent Living who are new to the transition process.
Learning Objectives
Upon completion of this webinar, participants will have knowledge and resources that will enable them to:
- Describe how IL philosophy and CIL staff support a successful transition process by reinforcing the peer-to-peer approach
- Identify skills and services most often needed by individuals who are transitioning out of an institution and into the community
- Explain the process for assessing an individual’s training needs prior to and after the transition
- Assist individuals to develop a personalized training plan to ensure their successful transition and integration into community living
